Any insights on what the difference is between digital cable that requires a cable box vs. digital tv standards?

I'm wondering why I can't just connect my digital cable directly to my new HDTV with a digital tuner - I have to rent a box from cable company. Why don't they just transmit the same Clear QAM signal over cable that my TV uses? Is there a mandate to do so?
